Primary Connections for Youth and Families (PCYF)

Our team is seeking adolescents aged 12-17 and their caregivers to participate in a brief research study. Eligible participants may be approached when attending primary care appointments at Lurie Children’s Hospital and offered the opportunity to participate. Participants in this study will be asked to answer questions about their health habits on an iPad, and may be asked to watch a brief educational video. Families will receive a $10 Amazon gift card for completing the first survey and may be eligible to receive up to $170 in gift cards for completing additional surveys.
